Job Summary:

The Outreach, Resource and Enrollment (ORE) Specialist will staff the WOW Mobile unit, facilitating visits via partner organizations to conduct one-on-one education, point-of-care testing, connection to HIV treatment, health education counseling regarding PrEP, and coordinate virtual or in-person sessions with NP providers for PEP or PrEP prescriptions. The ORE Specialist will also provide people visiting the WOW Mobile assistance with medical insurance, assistance signing up for Ready/Set/PrEP or other Patient Assistance programs, and other community services that can help with income assistance, housing, utility bills, legal issues, etc. The ORE will be key to helping patients to adhere to PrEP and to remain in HIV treatment and reach viral suppression. The ORE will perform patient recalls for lapses in care and contact patients and/or do home visits to provide them with support and encouragement to fill prescriptions, take medications, and attend required medical appointments. The ORE may act as a liaison between the provider and the patient to support the clinical plan of care. The ORE Specialist increases health insurance coverage and access to quality primary health, dental, and behavioral care for community residents and established patients. The ORE will provide outreach and education for community residents and patients, who lack access to healthcare and health insurance, provide enrollment and application assistance to uninsured and underinsured people and build enrollment-focused partnerships with local groups and organizations.

Team members are expected uphold the health center mission by assuring that patients receive health care that is competent, caring, and meets with a high degree of satisfaction. To uphold the mission and beliefs of the organization by performing duties that promotes the health centers via the WOW mobile. In collaboration with primary care, behavioral health and social services, to provide supportive services to individuals, families or groups and assist with resource coordination in responding to the needs of patients and the facility.

In addition, staff must support FPCN's commitment to the creation of a trauma-informed system of care that continually recognizes and responds to the impact of traumatic stress on all those who have contact with the organization, including children, adults, families, caregivers, and staff.

Responsibilities/Duties:

  • Provide enrollment assistance (including but not limited to completing coverage applications, gathering required documentation and troubleshooting the enrollment process) for uninsured children and adults to access subsidized, low-cost and free health insurance programs through the PA health insurance marketplace (Pennie), Medicaid, Medicare, and the Children’s Health Insurance Program (CHIP). This includes advocating for people wrongly denied for these benefits.
  • Provide structured patient education on health coverage, engage in follow-up conversations and offer renewal assistance for enrolled individuals.
  • Advocate for retroactive health coverage for FPCN and non-FPCN visits to reduce client medical debt and recover revenue for the network.
  • Support the development and implementation of organizational and community based outreach and enrollment initiatives.
  • Distribute outreach materials to patients, community members, partner organizations and businesses to build coverage option awareness.
  • Develop, discover and attend community events in order to promote coverage options and the mission and services of the organization.
  • Collaborate with various local organizations to build awareness of coverage options, spur enrollment and build referral linkages.
  • Attend and actively participate in monthly O&E team meetings.
  • Attend and successfully complete all required training programs; participate in ongoing conference calls, webinars, and other professional development opportunities.
  • Accurately and promptly document all client engagement and outcomes in electronic health records system (Athena) to assist in case management, track goal achievement and maintain client satisfaction
  • Other duties as assigned.

ESSENTIAL DUTIES ADDENDUM:

Community Resource Duties:

  • Meet inreach goals, including minimum of 100 encounters per month
  • Meet outreach goals, including minimum of 1 outreach event per quarter
  • Meet enrollment goals, including minimum of 20 applications per month
  • Lead the development and implementation of outreach initiatives at your site

Required Education:

  • Bachelor’s degree in a related field (social work, public health, human services, etc.) preferred or equivalent experience with willingness to grow in the field

Required Experience:

  • Knowledge of the health and human services infrastructure, health insurance programs and public coverage options a plus
  • Knowledge of HIV treatment & PrEP a plus

Required Qualifications:

  • Ability to advocate and navigate complex benefits programs for patients
  • Ability to travel
  • Must be able and willing to travel around the health center’s service area either via a personal vehicle or public transportation (travel expenses reimbursed).
  • Strong computer skills with proficiencies in Outlook, Word, PowerPoint, Excel, internet-based applications and the Microsoft operating system.
  • Strong oral and written communication skills

Program Summary:

Family Practice and Counseling Network, a program of Resources for Human Development, is a network of community health centers providing comprehensive primary care, dental and behavioral health services. The Network consists of four health centers in Philadelphia – Abbottsford-Falls Family Practice & Counseling in the Northwest, the Stephen and Sandra Sheller 11th Street Family Health Services of Drexel University in the North, the Health Annex in the Southwest, and Annex West Health Center in West Philly housed in Action Wellness building (formerly Action AIDS), as well as the SBHC Collaborative, a partnership between Education Plus Health (EPH) and FPCN at CB School , B21 & Vaux, and Belmont Schools. Today the Network serves more than 20,000 patients annually and is an NCQA certified Patient-Centered Medical Home, and recognized by HRSA as a National Quality Leader for Behavioral Health.

The Network’s mission is to provide person-centered, integrated and comprehensive health services to individuals and families across their lifespan. The Network promotes resiliency and well-being among patients, staff and surrounding communities. Network services are targeted to the surrounding communities and the advisory committee that guides services primarily consists of people from the communities served.

RHD employees and programs in the City of Philadelphia are required to follow the COVID vaccine mandate. The City of Philadelphia has issued an Emergency Regulation Governing the Control and Prevention of COVID-19 Mandating Vaccines for Healthcare Workers and In Higher Education, Healthcare, and Related Settings (“Vaccine Mandate Regulation”), which mandates vaccine for healthcare workers and in higher education, healthcare, and related settings. This mandate became Effective on August 16, 2021 and requires Healthcare Workers and certain workers at Healthcare Institutions to be fully vaccinated by October 22, 2021. Fully vaccinated means it has been two weeks since the final dose of vaccine. Thus, we now require vaccination prior to start for all new hires.

COVID-19 considerations:
We are a health center, and follow all CDC guidelines. We provide PPE such as gloves and masks, as well as plexiglass barriers. We are continuing to require that staff wear masks to keep everyone safe. We also require vaccination for all new hires.
