Team members are expected uphold the health center mission by assuring that patients receive health care that is competent, caring, and meets with a high degree of satisfaction. In addition, staff must support FPCN's commitment to the creation of a trauma-informed system of care that continually recognizes and responds to the impact of traumatic stress on all those who have contact with the organization, including children, adults, families, caregivers, and staff.
- Provide social influencers of health assessments for all patients and link to appropriate services. This includes benefits counseling and advocacy to assure they receive and understand eligible services.
- Work collaboratively with the Primary Care staff, the Behavioral Health Providers, the Dental Providers, and the Supportive Service Providers to comprehensively assess needs of patients, create a care plan, and follow through to ensure service efficacy and efficiency.
- Monitor and evaluate patient progress as described in care plan and modify as needed.
- Assist in increasing positive health outcomes for patients via coordination and participation in health education and promotion activities in collaboration with the Primary Care Coordinator, Outreach & Enrollment, Community Engagement, and the Center Director.
- Upon referral and in collaboration with the care team, refer patients to community resources to provide access to services such as home care services, financial assistance, legal aid and services that address other social influencers of health.
- Act as a liaison for the medical legal partnership lawyer from LCD and staff and providers at the center.
- Recruit, hire, train, and provide reflective supervise for the Community Health Worker and AmeriCorps Member including services, scheduled, responsibilities and work flow.
- Attendance and participation in meetings, in-services, educational programs and other activities as deemed appropriate.
- Maintain current knowledge on available community resources for patients.
- Document all patient interactions, meetings, and sessions within Centricity/Athenahealth.
- Maintain current Health Center policies and guidelines.
- Other duties and responsibilities as assigned.
- Masters in Social Work required
- 1-2 years experience in a health care setting
- 1-2 years of supervisory experience
- Licensed Social Worker (LSW) preferred
- Excellent and effective organizational and time management skills to manage large patient volume and follow through
- Excellent interpersonal skills and ability to be a team player
- Proficiency in computer skills, familiarity with Electronic Medical Records and willingness to learn all computer systems
- Demonstrates initiative and leadership.
- Unconditional ability to maintain HIPAA regulations.
- Demonstrates cultural competency among persons from various ethnic and social backgrounds
- Professionalism, responsible, courteous and reliable
- Exhibit behaviors that align with RHD/FPCN’s organizational values, mission, and beliefs and avoid displaying behaviors that are in opposition to those values
